How do I sign up?
Pay the enrollment fee and first month plan cost and receive the associated benefits immediately.
What can I expect on my first visit?
We are excited to have you as our patient! On your first visit, pay the one-time enrollment fee and first monthly payment and receive the included benefits of the plan you choose. Most plans include a comprehensive exam, x-rays, and a professional cleaning
What should I do if I have periodontal disease?
We have a Perio Plan specifically designed for you. It includes your regular perio maintenance visits as well as additional discounts on specific perio services. During your initial visit, we will let you know if these services are needed.
What forms of Payment are accepted?
Credit and Debit cards including MasterCard, Visa, American Express, and Discover are accepted. You can also use your HSA card.
What Preventive Care is included?
Your preventive regular care includes a doctor exam, annual x-rays and a professional cleaning. These are included at no additional charge and are available based on the visit frequency you choose when you sign up for your plan.
What other discounts are included?
Your plan includes discounts on any restorative work that needs to be completed, (filling, root canal, crown, etc.) or on any elective work you may desire (implant, veneer, bleaching etc.). This saves you money when work is needed.
How do I receive a discount on procedures?
Just schedule the work you need and we will automatically apply the discounts. There is no paperwork or hassle to receive the valuable discounts.Is there a waiting period for benefits and services?No! There are no limitations or waiting periods for current or future dental needs.
How Do I Cancel My Membership?
We plan on your membership continuing on a month-to-month or yearly basis for as long as you have teeth. If you need to cancel at anytime however, give us a call and we would be happy to discontinue your plan
